Description
O presente relatório de prática clínica tem como finalidade a obtenção do grau de Mestre em Cuidados Paliativos. Enquadra-se no âmbito do 8º Mestrado em Cuidados Paliativos da Escola Superior de Saúde Dr. Lopes Dias (ESALD) do Instituto Politécnico de Castelo Branco (IPCB).
Pretende-se através do relatório refletir sobre os conhecimentos e competências adquiridas ao longo da aprendizagem teórica e consolidados durante a prática clínica, encontrando-se dividido em três capítulos: contextualização da prática clínica, aquisição de competências segundo os pilares dos cuidados paliativos e projeto de intervenção.
No primeiro capítulo é apresentado uma reflexão sobre o modelo de Cuidados Paliativos bem como a caracterização e organização do local de estágio e do serviço onde foi implementado o projeto de intervenção.
A reflexão das competências adquiridas enquadra-se no segundo capítulo, sintetizando os objetivos previamente delineados e as atividades realizadas com base nos quatro pilares dos Cuidados Paliativos e na reflexão de casos clínicos.
O projeto de intervenção, descrito no terceiro capítulo, centra-se na temática “Complexidade Psicossocial em Cuidados Paliativos”, sendo que a pertinência do tema surge da análise SWOT realizada no âmbito do indicador de qualidade referente à necessidade da existência de protocolos. A definição de Complexidade ainda não é consensual, contudo, nos últimos anos, tem emergido a investigação sobre esta temática como tentativa de organização dos cuidados. Também inerente à Revisão Sistemática da Literatura realizada no primeiro ano do Mestrado, foram identificadas algumas lacunas na investigação sobre Cuidados Paliativos em Portugal, nomeadamente a inexistência de publicações de guias de boas práticas deste tipo de cuidados. Neste sentido, foram desenvolvidas duas ações de formação, uma dedicada à sensibilização e formação básica em Cuidados Paliativos com intuito de sensibilizar e dotar os profissionais de saúde de um hospital com competências básicas e uma segunda ação dirigida ao serviço de Cuidados Paliativos, onde se fomentou a reflexão sobre o conceito de complexidade e, em particular, os principais aspetos da Complexidade psicossocial, permitindo identificar critérios de referenciação e monitorização dos mesmos através de um instrumento validado para a População Portuguesa. Este projeto ambiciona uma melhoria de qualidade dos serviços prestados pela equipa.
The purpose of this clinical practice report is to obtain a Master’s degree in Palliative Care. It falls within the scope of the 8th Master's Degree in Palliative Care at the Escola Superior de Saúde Dr. Lopes Dias (ESALD) of the Polytechnic Institute of Castelo Branco (IPCB). It is intended through the report to reflect on the knowledge and skills acquired during theoretical learning and consolidated during clinical practice, being divided into three chapters: contextualization of clinical practice, acquisition of skills according to the pillars of palliative care and project of intervention. On the first chapter is presented a reflection on the Palliative Care model as well as the characterization and organization of the internship place and the service where the intervention project was implemented. The reflexion of the acquired competences fits within the second chapter, summarizing the previously outlined objectives and the activities carried out based on the four pillars of Palliative Care and the reflexion of clinical cases. The intervention project, described in the third chapter, has its focal point on the theme “Psychosocial Complexity in Palliative Care”, where the relevance of the theme arises from the SWOT analysis carried out within the domain of the quality indicator referring to the need for the existence of protocols. The definition of Complexity is still not consensual, however, in recent years, research on this topic has emerged as an attempt to organize care. Also inherent to the Systematic Review of Literature carried out in the first year of the Master's, some gaps were identified in the research on Palliative Care in Portugal, namely the lack of publications of good practice guides in this type of care. Along these lines, two training actions were developed, one dedicated to raising awareness and basic training in Palliative Care with the aim of sensitizing and providing health professionals in a hospital with basic skills and a second action designed to the Palliative Care service, where reflexion on the concept of complexity was encouraged and, in particular, the main aspects of psychosocial complexity, making it possible to identify criteria for referencing and monitoring them through a validated instrument for the Portuguese population. This project has per ambition the improvement of the quality of the services provided by the team.
