Description
O presente relatório de estágio insere-se no âmbito do 8º Mestrado em Cuidados Paliativos da Escola Superior de Saúde Dr. Lopes Dias do Instituto Politécnico de Castelo Branco (IPBC). Este tem como finalidade a reflexão da prática clínica, a demonstração de competências adquiridas nas áreas-chave dos cuidados paliativos e a descrição detalhada das atividades desenvolvidas no estágio e no projeto de intervenção/plano de formação. A parte prática inerente ao estágio decorreu na Unidade de Cuidados Paliativos do Hospital X, durante o mês de março de 2020 e abril de 2021. Este estágio foi interrompido devido à pandemia, após estabilização da mesma retomei o estágio e completei as 300 horas de estágio. Os cuidados paliativos têm um papel crucial na vida de todos os doentes com uma doença crónica, avançada e incurável. A reflexão crítica nesta área foi sustentada através da apresentação de um caso clínico demonstrando os objetivos atingidos e as competências adquiridas consolidadas através dos conhecimentos lecionados ao longo de todo o meu percurso curricular. O tema apresentado teve por base estudos que demostravam que para além da importância da desprescrição, evidenciavam o seu benefício aquando do diagnóstico de Últimos Dias e Horas de Vida (UDHV), atendendo a que este é um período complexo e delicado com grande impacto na equipa multidisciplinar que exige intensificação dos cuidados ao doente e família. Assim, é pertinente demonstrar a importância da desprescrição nos últimos dias e horas de vida para o doente, identificar os obstáculos à desprescrição nos últimos dias e horas de vida e reconhecer as suas vantagens e benefícios. Estes foram alguns dos objetivos que me motivaram na escolha deste tema bem como o facto de constatar diariamente com esta realidade enquanto profissional de saúde.
This present internship report is part of the 8th edition Master's Degree in Palliative Care at the Escola Superior de Saúde Dr. Lopes Dias of the Instituto Politécnico de Castelo Branco (IPBC). It has as purpose is the reflection of clinical practice, the demonstration of skills acquired in palliative care key areas, and the detailed description of the activities developed in the internship and in the intervention project/action plan. The practice inherent to the internship took place in the Palliative Care Unit of Hospital X, during March 2020 and April 2021. This internship was interrupted due to pandemic, and after stabilization, I restarted the internship and completed the 300 hours of internship. Palliative care plays a crucial role in lives of all patients with a chronic, advanced, and incurable disease. The critical reflection in this area was sustained trough the presentation of a clinical case demonstrating the objectives achieved and the skills acquired and consolidated through the knowledge taught in my curricular pathway. The theme presented was based on studies that demonstrated the importance of deprescribing, they revealed its benefit during the diagnosis of Last days and hours of life/Últimos Dias e Horas de Vida (UDHV), having into account this is a complex and delicate period with great impact on the multidisciplinary team that requires intensified care to the patient and family. Thus, it is pertinent to demonstrate the importance of deprescribing in the last days and hours of life for the patient, identify the obstacles on deprescribing in the last days and hours of life, and recognize its advantages and benefits. These were some of the goals that motivated me in selecting this topic, as well as the fact that I see this reality on a daily basis as a health professional.
