O presente relatório é realizado no âmbito do 8º Mestrado de Cuidados Paliativos da Escola Superior de Saúde Dr. Lopes Dias do Instituto Politécnico de Castelo Branco, como parte integrante dos requisitos necessários à obtenção do grau de mestre em Cuidados Paliativos. Associado aos conhecimentos adquiridos ao longo da aprendizagem teórica, pretende-se assim descrever de forma reflexiva e crı́tica todo o percurso desenvolvido no contexto da Prática Clı́nica e assistencial junto de uma Equipa especializada que levou à aquisição de competências indispensáveis à prática de Cuidados Paliativos.
Este documento encontra-se dividido em três capítulos. O primeiro capítulo visa caracterizar o estágio de 300 horas realizado num Departamento de Cuidados Paliativos de uma Unidade Local de Saúde da Região Norte, descrevendo-se modelo de organização daquela unidade, tipologia e funcionamento, sua caracterização e análise crítica.
No segundo capítulo pretende identificar-se as competências adquiridas nas áreas-chave dos Cuidados Paliativos, durante a realização deste estágio: comunicação, controlo de sintomas, apoio à família, acompanhamento em situação de últimos dias de vida, apoio no luto e trabalho em equipa. São enunciados os objetivos e a forma como foram atingidos, pormenorizando-se as atividades realizadas para a sua concretização. Além disso, é feita a análise de um caso clı́nico que despoletou a discussão em equipa e a reflexão sobre as diferentes competências a adquirir.
O terceiro capítulo descreve o projeto de formação e de intervenção desenvolvido no serviço onde decorreu o estágio, intitulado “Apoio Domiciliário em Cuidados Paliativos Pediátricos: Complementar Cuidados para fazer a Diferença”, direcionado a potenciar a capacidade de resposta dos profissionais da unidade em situações de suporte em idade pediátrica, incluindo não apenas o apoio à criança em situação de doença limitante e ameaçadora de vida, como ao seu núcleo familiar. É feita a identificação da área problema, a análise de SWOT, os objetivos, e o plano pedagógico da formação e de intervenção, fundamentando-se as diferentes etapas, desde o planeamento, à realização e avaliação. Inclui, ainda, a revisão sistemática da literatura “Impacto dos Cuidados Paliativos Pediátricos domiciliários na qualidade de vida de crianças com DCC e dos seus cuidadores informais: revisão sistemática” que constituiu o primeiro estudo deste tipo conhecido, quer a nível nacional, quer internacional. Apesar da escassez de estudos, da heterogeneidade metodológica e do elevado risco de viés dos resultados, os achados sugerem que o domicílio pode ser um local apropriado de cuidados, parecendo existir impacto positivo das equipas domiciliárias de CP Pediátricos no aumento da qualidade de vida de crianças e cuidadores, assim como no controlo de sintomas, redução da sobrecarga e nos níveis de satisfação dos cuidadores.
Adicionalmente, e tendo a Prática Clínica sido interrompida por força do contexto de pandemia COVID-19, é feita uma reflexão ao longo dos diferentes capítulos sobre o seu impacto na organização do serviço, forma de abordar e cuidar dos doentes e suas famílias e na necessidade de reajuste do projeto de formação e intervenção inicialmente proposto.
O segundo ano do Mestrado em Cuidados Paliativos permite, desta forma, aplicar e consolidar os conhecimentos adquiridos no primeiro ano, assim como contribuir para o desenvolvimento do saber sobre os Cuidados Paliativos em Portugal.
Abstrac: The present report is done within the ambit of the 8th Master of Palliative Care at the Escola Superior de Saude Dr. Lopes Dias of Instituto Politécnico de Castelo Branco, as an integral part of the requirements necessary to obtain the master’s degree in Palliative Care. Associated with the proficiency acquired during the theoretical learning, it is our purpose to describe, in a reflexive and critical analysis,the entire course developed in the context of Clinical and Care Practice with aspecialized Team who led to the acquisition of skills essential to the practice of Palliative Care. This document is divided into three parts. The aim of the first part is to characterize the 300 hours internship carried out in a Palliative Care Unit of a Local Health Unit in the North Region, describing the unit's organization model, typology and functioning, its characterization and critical analysis. In the second part, the aim is to identify the skills acquired in the key areas of Palliative Care, during this internship: communication, symptoms control, family support, monitoring during the last days of life, support in mourning and teamwork. The objectives and the way in which they were attained are stated, detailing the activities carried out to achieve them. In addition, it is performed an analysis of a clinical case that activated the team discussion and reflection on the different skills to be acquired. The third part describes the training and intervention project developed at the service where the internship took place entitled “Pediatric Palliative Home Care: complementing care to achieve the difference”, aimed at enhancing the response capacity of the unit professionals to pediatric age supportive situations including, not only support to children in a situation of limiting and life-threatening illness, but also to the family core. It is identified the problem area, the SWOT analysis, the objectives, and the pedagogical plan for training and intervention, based on the different stages, from planning to realization and evaluation. It also includes a systematic review of the literature “Impact of Pediatric Home Palliative Care on the quality of life of children with complex chronic disease and their informal caregivers: systematic review”, which became the first national and international study, focusing this theme. Despite the scarcity of data, the methodological heterogeneity and the serious risk of bias, the findings suggest that home can be an appropriate place of care, and there seems to be a positive impact of Pediatric Palliative Home Care teams in increasing the quality of life of children and caregivers, as well as in the control symptoms, burden reduction and caregiver satisfaction levels. Additionally, and since Clinical Practice was interrupted due to the COVID-19 pandemic, a reflection was carried out throughout the text on its impact on the service organization and on approach and care to patients and their families and the need to readjust the initially proposed intervention project. The 2nd year for Master’s degree in Palliative Care allows, in such a way, to apply and consolidate the knowledge acquired in the 1st year, as well as to contribute for the development of knowledge on Palliative Care in Portugal.
