Relatório de Estágio apresentado à Escola Superior de Saúde Dr. Lopes Dias do Instituto Politécnico de Castelo Branco para cumprimento dos requisitos necessários à obtenção do grau de Mestre em Cuidados Paliativos,
Integrado no 7º Mestrado em Cuidados Paliativos da Escola Superior de Saúde , serve o presente relatório para descrever, relatar e refletir, crítica e pormenorizadamente, sobre todas as actividades preconizadas em Projeto de Prática Clínica e Relatório, realizadas na Unidade Curricular de Prática Clínica. Tal é um requisito para obtenção do grau de mestre. A prática clínica foi realizada numa Unidade de Cuidados Paliativos, num total de 300 horas. O Estágio foi realizado sob Orientação Pedagógica Científica da Professora Doutora Ana Paula Gonçalves Antunes Sapeta. No presente relatório cruza-se a observação direta e participante com a evidência científica, descreve-se o modelo de organização e funcionamento da equipa de Cuidados Paliativos, aprofundando a tipologia de prestação de cuidados e características do serviço; refletem-se as competências adquiridas nas áreas-chave dos Cuidados Paliativos, especificamente no trabalho em equipa, comunicação família; e, demonstra-se a aquisição de competências na estruturação e implementação de programas em Cuidados Paliativos. Posteriormente apresento o Projeto de Intervenção e Formação, que serão implementados numa Unidade de Cuidados Paliativos. O Projeto de Intervenção é um Projeto de Melhoria da Qualidade e consiste em implementar a realização de conferências familiares, de forma programada e estruturada, e de um plano de apoio ao luto, conforme preconizam Indicador de Qualidade para os Serviços de Cuidados Paliativos e que a eles se referem. O Projeto de Formação consiste em fazer Formação à Equipa multidisciplinar da mesma Unidade, sobre o Projeto de Intervenção e sobre o tema “Conferência familiar e luto na família". O Projeto de Intervenção inclui uma análise das “forças, fraquezas, oportunidades e ameaças” (SWOT), as ações a curto, médio e longo prazo e a forma de avaliação da melhoria do Projeto de Intervenção, com a respetiva construção dos Indicadores de avaliação. O Projeto de Formação, inclui os objetivos plano de sessão (conteúdos, metodologia, recursos, diapositivos da sessão de formação). É apresentada uma Revisão Sistemática da Literatura, sobre os temas dos Projetos de Intervenção e Formação "Conferência familiar e luto na família".
