DIssertação apresentada à Escola Superior de Saúde Dr. Lopes Dias do Instituto Politécnico de Castelo Branco para cumprimento dos requisitos necessários à obtenção do grau de Mestre em Cuidados Paliativos, realizada sob orientação científica da Mestre Ângela Sofia Lopes Simões, Professora Adjunta Convidada da Escola Superior de Saúde Dr. Lopes Dias do Instituto Politécnico de Castelo Branco.
Os Cuidados Paliativos definem-se como cuidados ativos e totais, prestados por equipas multidisciplinares com formação e treino adequado aos doentes e suas famílias, com doença crónica, avançada e que leva à morte. A complexidade destes doentes exige aos profissionais de saúde preparação técnica, formação teórica e experiência prática efetiva em equipas certificadas e de referência em Cuidados Paliativos. Assim, a formação diferenciada destes profissionais é essencial para garantir organização, excelência e qualidade dos cuidados prestados. No contexto do Mestrado em Cuidados Paliativos da Escola Superior de Saúde Dr. Lopes Dias, do Instituto Politécnico de Castelo Branco, o relatório enquadra-se assim, numa vertente profissionalizante, onde a componente prática foi realizada na Equipa Comunitária de Suporte em Cuidados Paliativos de A., com um total de 300 horas. A realização da prática clínica teve como objetivo geral a aquisição de competências nas quatro áreas chave dos Cuidados Paliativos nomeadamente: Comunicação Adequada, Trabalho em Equipa, Controlo de Sintomas e Apoio à família. A aquisição de competências para o desenvolvimento de uma comunicação adequada ocorreu através da análise e reflexão crítica dos momentos experienciados no decorrer da prestação dos cuidados ao doente e sua família, cujo intuito era a relação de ajuda para o alívio do sofrimento. No controlo de sintomas apliquei competências na avaliação das necessidades do doente e sua família: tais como: avaliação, monitorização e controlo de sintomas com recurso à aplicação de medidas farmacológicas e não farmacológicas, refletindo o seu benefício em prol do melhor para o doente. No trabalho em equipa os meus objetivos prenderam-se primeiramente com a integração na equipa multidisciplinar, seguido da aprendizagem e treino de estratégias de trabalho nomeadamente participação na prestação de cuidados ao doente e sua família e participação ativa na realização de reuniões de equipa dando o meu contributo como enfermeira com formação avançada na área. No apoio à família evidencio a conferência familiar e o apoio no luto, como inclusão no plano de cuidados, considerando a conferência familiar como um instrumento fundamental no apoio à família na gestão realista das expetativas do doente e sua família. A área temática do presente relatório foi desenvolvida com o intuito da aquisição de competências pedagógicas e científicas no âmbito da comunicação. Sendo a comunicação um dos pilares dos Cuidados Paliativos, que funciona como estratégia terapêutica de intervenção no sofrimento e controlo dos sintomas associados à doença avançada e terminal, torna-se pertinente fazer uma revisão sistemática da literatura sobre Gestão das Expetativas Realistas do Doente pós-Acidente Vascular Cerebral. E, por fim, dar o meu contributo para a melhoria contínua da formação e da qualidade da equipa através da realização de formação sobre: Protocolo da Transmissão das Más Notícias. Considero, que uma enfermeira com formação avançada em Cuidados Paliativos, desenvolvi competências no decorrer da prática clínica através do treino, análise e reflexão com o intuito de atingir/alcançar resultados de excelência ao nível da assistência ao doente e família através da avaliação continua, global e holística baseando-me na evidência científica.
ABSTRACT The Palliative Cares are defined as active and total cares, rendered for you equip multidisciplinary with formation and appropriate training to the patients and their families, with chronic disease, advanced and that takes to death. The complexity of these sick ones demands to the professionals of health technical preparation, theoretical formation and practical experience executes in you equip certified and of reference in Palliative Cares. Like this, these professionals´ differentiated formation is essential to guarantee organization, excellence and quality of the rendered cares. In the context of the Master’s degree in Palliative Cares of the Higher School of Health Dr. Lopes Dias, of the Polytechnic Institute of White Castle, the report is framed like this, in a vocational slope, where to practical component it was accomplished in the it Equips Community of Support in Palliative Cares of A., with a total of 300 hours. The accomplishment of the clinical practice had as general objective the acquisition of skills in the four areas key of the Palliative Cares namely: Appropriate Communication, The Work in it Equips, Control of Symptoms and Support to the Family. The acquisition of skills for the development of an appropriate communication happened through the analysis and critical reflection of the moments experienced in elapsing of the installment of the cares to the patient and his family, whose intention was the relationship of help for the relief of the suffering. In the control of symptoms applied skills in the evaluation of the needs of the patient and his family: such as: evaluation, monitoring and control of symptoms with resource to the application of pharmacological measures and no pharmacological, reflecting their benefit on behalf of the best for the patient. In the work in it equips my objectives were arrested firstly with the integration in the it equips multidisciplinary, following by the learning and training of work strategies namely participation in the installment of cares to the patient and his family and participation activates in the accomplishment of meetings of it equips giving my contribution as nurse with formation moved forward in the area. In the support to the family evidence the family conference and the support in the mourning, as inclusion in the plan of cares, considering the family conference as a fundamental instrument in the support to the family in the realistic management of the expectations of the patient and his family. The thematic area of the present report was developed with the intention of the acquisition of pedagogic and scientific skills in the context of the communication. Being the communication one of the pillars of the Palliative Cares, that works as therapeutic strategy of intervention in the suffering and control of the symptoms associated to the advanced and terminal disease, it is becomes pertinent to do a systematic revision of the literature on Management of Realistic Expectations of the Patient Cerebral Vascular Post-Accident. And, finally, to give my contribute for the continuous improvement of the formation and of the quality of the it equips through the formation accomplishment on: Protocol of the Transmission of the Bad News. I consider, that as nurse with formation moved forward in Palliative Cares, I developed skills in elapsing of the clinical practice through the training, analysis and reflection with the intention of reaching / reach excellence results at the level of the attendance to the patient and family through the evaluation continues, global and holistic basing on the scientific evidence.
